When your patients need prompt, attentive mental health care, Urgent Behavioral Health Care is there.

One of our practice’s top priorities is maintaining a patient’s health after discharge from psychiatric inpatient care. When a patient is hospitalized for a condition related to chronic mental illness, we understand how important it is for them to receive prompt care upon discharge. Our clinic prioritizes these patients so we can see them within the required 7 to 10 day timeframe after they leave the hospital.

We know that an effective discharge is critical to a patient’s recovery and stabilization. We also know that the highest rate of re-hospitalization happens within the first 30 days after discharge from the hospital, with medication management being of critical importance.

One of our staff members is available 24-hours a day for urgent issues. If you’re discharging a patient after hours, call us and speak to one of our staff members directly to set up the patient’s first follow-up appointment. Once your patient is stabilized, they can return to you for continued care or we can provide ongoing treatment and medication management, making sure you’re informed on your patient’s progress with us.

As a multidisciplinary psychiatric practice, we offer wraparound services, including a licensed professional counselor and licensed clinical social worker on staff, to provide a continuum of care for your patient.
